EZ-NET News and Updates

- Please submit all authorizations and attach clinical records through EZ-NET.
- Please do not submit via fax unless you cannot access EZ-NET.
- If you are unableto find a member on EZ-NET, use the portal to complete an eligibility request form.
- If a member is not listed on EZ-NET, do not fax an authorization request. Our team must confirm eligibility first.
- Please submit a retro auth if necessary.

Hoag Physician Partners EZ-NET Portal allows practices to:

- Look up members
- Look up in-network providers
- Submit authorizations
- Check authorization status
- Check claims status
- Access important documents


*** Important Update ***

The EZ-NET Provider Portal will sunset on 12/31/2021. 

Please be sure you are ready for the transition by requesting your new EpicLink login and completing training prior to 12/31/2021. 

You can visit www.hoag.org/hpp to request access and training.  Your login will be available for read only access of past authorizations through 2/28/2022. 

All new authorizations beginning on 1/1/2022 will be in the EpicLink portal.
